Supporting culturally tailored access to the Diabetes Prevention Program (DPP) and Diabetes Self-Management Education and Support (DSMES)
In July 2023, QARI began a 5-year project funded by the CDC to help advance prevention and management of diabetes in priority populations considered to be most at risk of developing type 2 diabetes. Working in close partnership with Dr. Tam Nguyen of Boston College, Joslin Diabetes Center’s Asian American Diabetes Initiative (AADI), South Cove Community Health Center, and Enhance Asian Community on Health (EACH), QARI plans to implement the Diabetes Prevention Program (DPP) to help prevent diabetes in high risk Asian American communities, and the Diabetes Self-Management Education and Support (DSMES) program to empower community members to learn how to support and manage their diabetes diagnosis. QARI is proud to be able to integrate its long-time social services with these important programs to provide wraparound solutions for immigrants in our community. Our program includes four key focus areas:

Strengthen self-care practices by improving access, appropriateness, and feasibility of diabetes self- management education and support (DSMES) services for priority populations.

Prevent diabetes complications for priority populations through early detection.

Increase enrollment and retention of priority populations in the National Diabetes Prevention Program (National DPP) lifestyle intervention and the MDPP by improving access, appropriateness, and feasibility of the programs

Support the development of multi-directional e-referral systems that support electronic exchange of information between health care, CBOs, and DPP/DSME providers
